ABSTRACT

Objective To investigate the molecular mechanism of colistin resistance mediated by mcr-1and pmrAB genes in clinical Escherichia coli ( E. coli) isolates. Methods A total of 1988 clinical E. coli isolates were collected from the First People′s Hospital of Shangqiu from 2010 to 2017 and screened for colistin-resistant isolates using agar dilution method. The minimum inhibitory concentrations ( MICs) of nine common clinical antibiotics were determined using broth microdilution method. PCR and sequencing analysis were performed to detect the colistin resistance genes of mcr-1 and pmrAB. Conjugation experiments were used to test the transferability of the plasmid carrying mcr-1 gene. S1-PFGE and Southern blot were used to locate the plasmid carrying mcr-1. All colistin-resistant E. coli isolates were typed by multilocus se-quence typing ( MLST) . Results Six colistin-resistant E. coli isolates were obtained by agar dilution meth-od. The results of susceptibility testing showed that all of the six isolates were multidrug resistant. PCR and sequencing analysis revealed that four out of the six strains carried mcr-1 gene, and the other two isolates both had an amino acid substitution (L167P) caused by pmrB gene mutation. The results of conjugation ex-periments, S1-PFGE and Southern hybridization analysis showed that the plasmids of four mcr-1 gene-posi-tive E. coli strains were located on a conjugative plasmid about 60 kb in length. MLST analysis classified the six isolates into six distinct sequence types ( STs) . Conclusion This study suggested that mcr-1 gene and mutations in pmrAB gene were the main mechanisms mediating the resistance of E. coli to colistin. In clinical practice, the occurrence and spread of colistin-resistant E. coli should be further monitored, and the rational use of antibiotics should be promoted to prevent the spread of colistin-resistant strains.

ABSTRACT

Objective To determine the frequency of depression in patients with Parkinson' s disease(PD) and healthy controls over 50 years of age and to analyze the characteristics and influencing factors of PD with depression(PDD) in Nanjing.Methods One hundred and twenty-six PD patients were diagnosed and assessed using Self-rating Depression Scale (SDS) and Hamilton Depression Scale (HAMD).The frequency,characteristics and influencing factors of depression were statistically analyzed,and the factor analysis of HAMD was carried out.Also,one hundred and twenty-four healthy subjects over the age of 50 were selected as the control group.Ressults The incidence of depression in PD group was 48.4% ( 61/126):15.1% (19/126) for mild depression,27.8% (35/126) for moderate depression,5.6% (7/126) for severe depression.The incidence of depression in the control group was 9.7% (12/124):5.7% (7/124) for mild depression,2.4% (3/124) for moderate depression,1.6% (2/124) for severe depression.There was a significant difference between these two groups( x2 =45.36,P < 0.01 ).Univariate and Logistic regression analysis revealed that a high frequency of depression occurred in patients with long PD duration,high H-Y stage and UPDRS Ⅲ.According to each factor analysis of HAMD,the scores of cognitive impairment,tardiness,anxiety and sleep disturbances of the PD patients with depressive syndromes were higher than that of the control group.Conclusion Depression is a relatively common complication of PD in Nanjing which is associated with long PD duration,severity of motor disturbances and increasing H-Y stage.

ABSTRACT

Objective To analyze the adverse drug reactions(ADRs)of edaravone in treatment of patients with acute cerebral infarction(ACI)by reviewing Chinese medical literature and to evaluate safety of edaravone treatment in ACI.Methods Publications from Pubmed and Chinese Biomedical Literature Datababe(CBMdisc)were reviewed and the ADR of edaravone was analyzed among the published 8645 cases.Literatures about randomed-control clinical trails(RCTs)on security of edaravone for treating ACI Was analyzed by meta-analysis.Results Abnormal hepatic function,especially mild elevation of aminotransferase,renal dysfunction and skin rash induced by edaravone were tIle most common ADRs.Among 8645 patients,ADRs were reported in 283(3.27 % ).The meta-analysis in RCTs showed that between the group treated with a combination of edaravone and routine and the group treated with routine treatment only,there was no significant difference in the occurrence rate of ADRs(OR=1.18,95 % CI0.70-2.00,P=0.536),elevation of aminotransferase(OR=1.23,95 % CI 0.57-2.68,P=0.595)or renal dysfunction including albuminutia,increased level of serum ereatinine and nitrogen(OR=1.65.95 % CI0.57- 4.79.P=0.353).Conclusion Edaravone has a low ADRs OCCurrence rate and iS safely used in cerebral infarction treatment.

ABSTRACT

Objective To study the effect of nutrient intake in different periods of gestation on fetus in-trauterine growth. Methods 239 cases of pregnant women were divided into three groups:(1)10~2 weeks; (2)21~30 weeks; (3)over 31 weeks. Nutrient intake was calculated by computer nutrition system. Fetus intrauterine growth retardance in different periods of gestation was estimated by routine methods. The incidence rate of fetus in-trauterine growth retardance between different periods of gestation and different nutrient intake was compared. Re-suits Group 1, group 2 and group 3 : comparison to cases with < 90 % RDA or > 110 % RDA, the incidence rate of fetus intrauterine growth retardance was higher than that in pregnant women with 110 % - 90 % RDA(P < 0.05).Conclusion Optimal nutrient intake was important to reduce the incidence rate of fetus intrauterine growth retar-dance.

ABSTRACT

Objective To investigate some water soluble vitamin nutrition status of old people in Shangqiu and explore the improving ways. Methods A dietary survey was carried out and urinary VB1, VB2 and VC were test-ed. Results The intakes of VB1 ,VB2 and VC were much lower than the recommended amounts. Conclusion The intakes of VB1 ,VB2 and VC were seriously deficient in old people and preventive measures should be taken such as intaking fresh vegitable, fruits and coarse food grain.

OBJECTIVE To evaluate the relationship between the amount of interleukin-8(IL-8) and granulocyte colony-stimulating factor(G-CSF) and Helicobacter pylori(Hp) infection in gastric mucosa.METHODS Enzyme linked immunosorbent assay(ELISA) was used to detect the amount of IL-8 and G-CSF in 56 patients including normal gastric mucosa(n=18) and H.pylori infection patients(n=38) who were performed with gastroscope with their supernate fluid of gastrostoma mucosa tissue culture,and to examine the difference between Hp infected cases and non-Hp infected ones and H.pylori infection patients before and after treatment.RESULTS The amount of IL-8 and GCSF in Hp infected cases was significantly higher than non-infected ones(P

ABSTRACT

Objective To study the best dose of batroxobin that protects forebrain ischemia reperfusion injury in gerbils.Methods 80 gerbils were randomly assigned to receive different dose of batroxobin (1 BU/kg, 2 BU/kg, 4 BU/kg, 8 BU/ kg, 16 U/kg or 32 BU/kg) in treatment group, NS in control group or nothing in sham-operation group. There were 10 gerbils in each group. NS (control group) or batroxobin (treatment group) was given intraperitoneally three hours before establishment of forebrain ischemia reperfusion model. Apoptotic cells were detected by TUNEL technique and the number of apoptotic cells in the hippocampal CA 1 territory was counted using Olympus microscope.Results The apoptotic cells in the treatment groups (8 BU/kg, 16 BU/kg, 32 BU/kg) markedly decreased than those in the control group and the other treatment groups (1 BU/kg, 2 BU/kg, 4 BU/kg) (all P0.05). Conclusions Batroxobin has a role of reducing neuron apoptosis after cerebral ischemia. The best dose of batroxobin that protects forebrain ischemia reperfusion injury in gerbils is 8 BU/kg.

ABSTRACT

Objective To study the influence of applied times of Batroxobin on neuroprotective effects in cerebral ischemic-reperfusion(IR) injury gerbils.Methods 45 gerbils were randomly divided into five groups:normal group;cerebral IR injury model group(IR group);three times group;five times group;seven times group.The later three groups were administered Batroxobin 8 U/kg by abdominal injection q.o.d with certain times respectively.The normal group and IR group were not given drug but isometric physiological saline.Flow cytometer was used to measure apoptotic neurons of the hippocampal CA1,meanwhile electronic microscope was used to detect the ultrastrctural change of the hippocampal CA1 region.Result The quantity of apoptotic neurons in the every Batroxobin groups were significantly less than those in the IR groups(all P0.05).The ultrastrctural changes in five times group were lighter than those in three times group,but no significant difference was found between five times group and seven times group.Conclusion Three times,five times and seven times of Batroxobin can reduce the number of apoptotic neurons after IR injury.However,applied five times and seven times of Batroxobin has stronger neuroprotective effect than three times.
